Template
provides easy methods for entering the headers, footer, name line,
and title of an examination. A section button allows quick demarcation
of an examination with different sections. The directions and formatting
of the following item types are provided: true-false, matching,
multiple-choice, and fill-in the blank. The question stem option
for the multiple-choice questions can be used for formatting short-answer
questions. |

As
in the Cognitive (Exam) Template, this template provides easy methods
for entering the headers, footer, name line, and title of a measure.
A section button allows quick demarcation of an examination with
different sections. The directions and formatting of the following
rating item types are provided: semantic differential, interval
scales, Likert-type scales, rating scales, and alpha/numeric scales.
A preferences dialog box allows multiple options to be selected
for each item type including reversing the rating scale, neutral
response, and undecided responses when appropriate for that form
of item. |

As
in the templates above, this template provides easy methods for
entering the headers, footer, name line, and title of rubrics. The
user is provided with options of how the rubric form is to be layed
out including places for names, multi-level/multi-trait rubrics.
The user has complete control over all aspects of the rows and columns
in the rubric using a specially designed toolbar. |

Download:
Click
on the template links above. If a dialog box to save the template
on your computer does not appear, either right-click or control-click
the template link. Make sure that the 3-letter suffix is .dot and
NOT .doc. Some browsers will save the file automatically as .doc.
Use the right-click or control-click method to manually reset the
.doc suffix to .dot.
Where
to Save:
To
enable use of the templates as MS Word® intends, create a new
folder inside the "Template" folder of the Microsoft Office®
folder. Name this folder something you will recognize such as "Assessment."
Save the templates directly into this folder. Make sure that the 3-letter
suffix is .dot and NOT .doc. Some browsers will save the file automatically
as .doc. Use the right-click or control-click method to manually reset
the .doc suffix to .dot.
How
to Use:
Open
MS Word® and open the project gallery. If necessary open the folder
in which the templates were placed. Then, double click the desired
template. A new document will be created with the associated toolbar
for the template visible. It is suggested that a little time be spent
trying out the various options on the toolbar to see how they work.
In music, we call this "practicing." In computers, we call
this "wasting time."
